WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK AGREEMENT

PLEASE READ CAREFULLY BEFORE SIGNING / AGREEING. THIS IS A BINDING LEGAL DOCUMENT THAT RELEASES FLYERS WRESTLING CLUB, LINDBERGH HIGH SCHOOL, AND RELATED ENTITIES FROM LIABILITY IN THE EVENT OF INJURY OR DAMAGE.

1. Acknowledgment of Risks

I, the undersigned parent or legal guardian of the participating minor athlete ("Participant"), acknowledge and fully understand that wrestling is a high-contact, physically demanding sport involving inherent risks. These risks include, but are not limited to:

• Physical injury, severe body harm, muscle strains, bone fractures, sprains, joint dislocations, concussions, skin infections, or paralysis.

• Accidental injury or illness occurring during practices, conditioning sessions, meets, tournaments, or traveling to and from club events.

• Actions, omissions, or negligence of other participants, coaches, referees, volunteers, or spectators.

I voluntarily choose to allow my child to participate in all activities associated with Flyers Wrestling Club ("FWC") and acknowledge that I am fully aware of these inherent risks.

2. Assumption of Risk

On behalf of myself, my spouse, the Participant, and our personal representatives, heirs, and assigns, I EXPRESSLY AND VOLUNTARILY ASSUME ALL RISKS, both known and unknown, associated with participation in Flyers Wrestling Club activities, practices, and sponsored events, even if arising from the negligence of the Releasees (as defined below).

3. Release and Waiver of Liability

In consideration of allowing the Participant to register and participate in Flyers Wrestling Club programs, I hereby release, waive, discharge, and covenant not to sue:

• Flyers Wrestling Club, its officers, directors, coaches, volunteers, employees, and agents;

• Lindbergh High School and Lindbergh Schools / School District, including its board members, administrators, employees, and staff;

• Flyers Wrestling Booster Club;

• Event hosts, sponsors, and venue owners/operators;

(collectively referred to as the "Releasees"), from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or related to any personal injury, bodily injury, illness, disability, death, or property damage sustained by the Participant or parent/guardian, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

4. Emergency Medical Treatment Authorization

In the event of an emergency, injury, or sudden illness during any practice, match, or event, I hereby grant permission to the coaches, staff, and representatives of Flyers Wrestling Club to authorize emergency medical evaluation, transport, and treatment for my child if I am unavailable or unable to be reached immediately. I agree that I am solely responsible for any medical expenses incurred as a result of such emergency treatment.

5. Medical Fitness & Insurance

I confirm that the Participant is in good health, physically fit, and has no undisclosed medical conditions or physical limitations that would prevent safe participation in wrestling activities. I confirm that the Participant is covered by primary medical/health insurance.

6. Severability

If any provision or portion of this agreement is held to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining terms shall continue in full legal force and effect.

By signing below (or checking the agreement box during digital registration), I confirm that I am the parent or legal guardian of the Participant, that I have read and fully understand this Waiver and Release of Liability, and that I agree to all of its terms voluntarily.
